6 Amityville, NY Amityville, NY Full-time Full-time $60,000 - $65,000 a year $60,000 - $65,000 a year Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County.
Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant.
Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations.
Hope For Youth is offering a $1000 sign on bonus for this position Hope For Youth's Adolescent Diagnostic and Respite Services Program (Diagnostic) provides respite, emergency and diagnostic services for up to 12 youth, co-ed, from ages 12-20.
Youth are referred through Nassau and Suffolk Department of Social Services, Probation and the Crisis Bed Network.
The Diagnostic Program Manager will oversee all case management services, aftercare services, prepare psychosocial assessments and diagnostic evaluations.
The Program Manger provides direct supervision to Assistant Program Manager, Case Worker and Case Worker Assistant.
The Program Manager is instrumental in maintaining the overall functions of the program with the assistance of the Assistant Program Manager.
The position offers the opportunity for some remote work within agency policy, including but not only completion of new hire introductory period and good standing.
To be a successful program manager you must be ready, able and willing to do the following always:
Function within the policies and procedures as outlined in the program practice guide and employee manual.
Provide overall management of assigned site.
Complete necessary reports and case documentation.
Liaise with DSS, schools and other child and youth collateral contacts.
Provide crisis intervention and counseling as needed.
Provide supervisees with the knowledge, skills and tools needed to succeed Adhere to state regulations and applicable code of conduct.
Preparing psychosocial assessments and diagnostic evaluations Create discharge plans and provide 6 months of aftercare services Conducting home visits as needed Perform all related tasks or other agency and/or programmatic required needs associated with this role Support other projects and residential program locations, as needed.
How you will generally spend your time in this job:
50% General casework responsibilities, documentation, reports 20% Supporting youth 20% Supporting staff 10% Staff meetings/supervision/training To qualify for this job you need to have and maintain:
LMSW required/ LCSW preferred 2 years' post-master's experience with child serving systems such as child welfare and juvenile justice Computer proficiency in Microsoft Office Suite, including MS Word and MS Excel and similar programs and third-party reporting portals NYS driver's license in good standing Access to own, reliable vehicle Ability to provide 24/7 on call availability during the week; on call rotation for weekend days Physical Requirements:
Operate a motor vehicle carrying up to ten (10) children and youth, lift/move up to fifty (50) pounds, escort children and youth into the community using stairs when necessary, restrain a child or youth during a crisis, read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements.
